It's a well-designed survey—well done!
However, there are a few technical issues:
- After indicating that a participant doesn't use any delivery services, they are still asked questions related to delivery services, such as how often they use it and what types of meals they order.
- It would be beneficial to include multiple-choice options for questions like the types of meals they order.
Additionally, I'd love to gain more insight into the purpose of the survey, your plans for using the results, and more details about your app. The nature of your app can significantly influence the questions you ask in the survey.
Good survey. Correct work with ranges of numeric values. And a good text at the end of the survey.
Some restrictions should be taken into account:
1) this platform does not support open questions, I cannot specify information in the "Other (Please specify: ______)" field.
2) the user can skip questions
Perhaps other tools can be more helpful and diversify your survey? For example, you can create open-ended questions in Google Forms.

I like to start with the positives. Good variety of questions. Asking one question at a time allows the user to focus on their answer and prevents distractions.
However, if this is a survey about Meal Delivery Services, why do visuals don't reflect that?
Next, I'm wondering, is there a specific need to ask a person for their demographical data? And if so, I'd recommend keeping options shorter. For instance. What is your gender? Male, Female, Other: ____ This way you allow users to self-identify.
Finally, most users don't complete long surveys. A progress bar or letting the user know the number of questions ahead is good practice for managing expectations.
Surveys are an immediate go-to for research, but asking the right questions at the right time is key. Hope this helps.
