Great job creating the survey, Vinay!
The only thing I would add is a question to understand what would be a reasonable price for subscriptions, and what would they want to be included in the subscription (daily meals, twice-a-week meals, priority on delivery, etc). So you can understand with what prices you can work, and create tiers for subscriptions.
Example:
Bronze: once-a-week meal, and discounts on extra meals.
Silver: twice-a-week meal, and discounts on extra meals.
Gold: four times a week meals, and discounts on extra meals.
Diamond: six times a week meals, discounts on extra meals, priority on delivery.
This can be an open question with some suggestions.
